Preston is a psychiatrist at Universal Hospital, she is a doctor of medicine as well as a member of the psychiatric community. She is diagnosed as having bipolar disorder, which is a mental illness that causes unusual, often destructive mood swings from extreme mania to deep depression. Dr. Preston is obviously frustrated with the idea that anyone would question her occupation as she clearly has a very important job that involves taking care of people who have manic-depressive disorders, or something similar.

When she responded by saying she was a psychiatrist it was an obvious attempt to put everyone at ease and show that she knows what she is talking about when it comes to being a mental health professional. Haley was obviously impressed by Dr. Preston's job title and wanted her to know how important she was so she decided to comment on it in hopes of giving her some confidence and making her feel good about herself.
