During World War I he was awarded the Military Cross and Distinguished Service Order for his services as a medical officer, and later received the Distinguished Service Order for bravery under fire. In October 1916 he was appointed consultant paediatrician to the Royal Air Force, becoming director of Midwifery at Craiglockhart Hospital near Edinburgh. In 1918, shortly after the war ended, Dr Pirie became medical adviser to the British Red Cross Society and from 1919 to 1922 he was Director of Research at the National Research Council of Canada.

In 1922 Dr Pirie became consultant medical officer with Great Britain's Olympic team and also with Great Britain's Land Forces in Palestine, being promoted to the rank of lieutenant colonel during this period. He retired from army service in 1926 and remained chief medical officer at Craiglockhart until 1932, when he took up a post as consulting physician to the British Red Cross Society until 1939.
