James McCay was born in 1828 in Scotland, and emigrated to Canada in 1832. He was baptized into the Church of Scotland, married in 1855, and raised a family. He obtained his medical degree in Scotland, immigrated to the United States, and became a member of the New York State Medical Society. During the Civil War he served as surgeon for the 9th Battery of the New York State Volunteers
At its conclusion he returned to his medical practice, which he continued until his death on August 12, 1895.
